Compare complete coverage paths, not two interchangeable products

Medicare Advantage and Medigap are not two versions of the same product. Medicare Advantage is another way to receive Medicare-covered benefits through a private plan that contracts with Medicare. Medigap is supplemental insurance designed to help pay certain costs in Original Medicare. A Medigap policy cannot be used to pay Medicare Advantage cost sharing.

The useful comparison is Medicare Advantage versus Original Medicare with any separate Part D drug plan and optional Medigap policy. Neither path is best for everyone.

Original Medicare with optional Medigap and Part D

Original Medicare generally lets you use any doctor or hospital in the United States that takes Medicare. Original Medicare does not have an annual out-of-pocket maximum for covered Part A and Part B services. A Medigap policy may help pay specified deductibles, copayments, and coinsurance, depending on the standardized plan. A separate Part D plan is usually needed for outpatient prescription coverage.

Medigap premiums vary even when policies use the same plan letter and core benefits. The federal Medigap Open Enrollment Period is a one-time six-month period that starts when you have Part B and are 65 or older. After it ends, an insurer may use medical underwriting unless a guaranteed-issue right or state protection applies.

Medicare Advantage combines coverage under plan rules

Medicare Advantage plans must cover Medicare-covered Part A and Part B services. Many include Part D and may include additional benefits. Plans may use provider networks, service areas, referrals, or prior authorization. They have an annual limit on what you pay out of pocket for covered Medicare services.

Plan availability, networks, drug lists, pharmacies, benefits, and costs can vary by county and can change each year. A low or zero additional plan premium does not mean health care is free; the Part B premium and plan cost sharing can still apply.

Use your own care and future flexibility to compare

List doctors, hospitals, prescriptions, pharmacies, travel patterns, and the way you prefer to budget costs. Verify providers with both the plan and the provider. Compare total expected costs, not only premiums. Read current plan documents and use Medicare Plan Finder for all options in your area.

Also consider future Medigap eligibility before leaving or changing coverage. California’s Birthday Rule helps people who already have a Medigap policy move to an equal-or-lesser-benefit Medigap policy during a 60-day period beginning on their birthday. It does not give every Medicare Advantage member a guaranteed right to buy Medigap. Confirm any guaranteed-issue right before changing coverage.

Three things to do next

  1. List your doctors, hospitals, prescriptions, pharmacies, and travel needs.
  2. Compare total costs and coverage rules, not only the monthly premium.
  3. Verify current plan details and future Medigap eligibility before changing coverage.
